Award-Winning Medical Care – VNA of Florida’s caliber of care is highly recognized nationwide, receiving the Homecare Elite Award five years in a row. In 2012 we received the Home Health Consumer Assessment of Healthcare Providers and Systems Honors (HHCAHPS), which could be called “The Patient’s Choice Award.” HHCAHPS surveys 1,800 homecare agencies across the country and honors those scoring in the top 20 percent of patient-satisfaction rankings.

We developed an innovative system for high-risk patients, partnering and coordinating with local hospitals to ensure our patients’ seamless transition from hospital to home. The results show. Our success rate preventing patients from requiring hospital readmission exceeds state and national averages.

Health insights and education empower patients and can prevent future tragedies. We arrange free stroke and cardiovascular-health support groups at area country clubs and civic halls, featuring medical experts as speakers and providing attendees with free dinners.

Emotional well being can translate into physical well being. Our Silver Socialites program tends to the social needs of seniors. Inspired by studies linking loneliness to early death, VNA of Florida launched Silver Socialites, which organizes activities that foster friendships, create memories and enhance overall health.

VNA of Florida offers the full spectrum of health services—from illness and injury rehabilitation including skilled nursing, physical-, speech- and occupational therapy to round-the-clock respite care. Our newly launched Visiting Physicians can provide patients the full spectrum of care they need until they can make it back in to see their primary doctor.

At VNA, it is our passion and goal to tend to the needs of our aging seniors. We take a progressive approach to senior care and actively seek new ways to help in an everchanging environment. Quality sssisted living is something we identified a need for in our communities and that is why we built Grand Oaks.
