Here you will find a list of frequently asked questions we’ve discussed with future residents of Grand Oaks.

 

How can I apply for assisted living?

Seniors seeking assisted living must complete the state required form AHCA 1823. This form necessitates that a health assessment be made of the prospective resident by his or her primary care physician. If the prospective resident does not have a primary care physician, he or she may have their health assessment performed by our visiting physicians.

 

Does Grand Oaks accept Medicare or Medicaid?

Medicare will not cover assisted living costs at any assisted living facility. Grand Oaks of Jensen Beach and Palm City do not accept Medicaid, however Grand Oaks of Okeechobee does accept Medicaid on a limited basis. Grand Oaks of Okeechobee uses third party agencies that pay for a portion of the resident’s assisted living costs and the resident pays for the remaining portion.

 

Are residents billed month to month or yearly?

Grand Oaks residents are billed on a month to month basis.

 

What methods of payment does Grand Oaks accept?

Residents may pay via ACH. ACH is an automatic debit system that is set up before a resident begins their stay at Grand Oaks. Future residents will set up a date and time to complete paperwork before their move-in date. This paperwork includes an automatic debit authorization form which requires future residents to provide a voided check with their account information; payments will be automatically debited from this account each month. How long is a Grand Oaks lease? What does it take to terminate a lease with Grand Oaks?

Grand Oaks offers year long leases. Residents may terminate a lease as long as he or she provides Grand Oaks administrator with a 30-day written notice.

 

Does Grand Oaks require a security deposit when reserving a room?

Grand Oaks requires a one-time non-refundable community reservation fee.

 

Does the assisted living room rate include levels of care?

No, the listed room price is a base rate. The level of care price is an additional cost added to the base room rate. The level of care price is subject to change based on a resident’s change of condition. If the Grand Oaks staff notices a change in a resident’s ADL or Activities of Daily Living, the resident may need a more intensive level of care which could subsequently raise the level of care price. This does not include Memory Care.

 

Is the rate for Memory Care all inclusive?

Yes, the Memory Care rate is all inclusive.

 

How does Grand Oaks determine a resident’s required level of care?

Residents undergo a functional assessment to determine their specific needs before beginning their stay at Grand Oaks. The assessment grades residents on ADL or Activities of Daily Living. For future residents, the assessment can be done before a commitment is made so future residents understand the costs that may be associated with their required level of care. This assessment will be completed by the Grand Oaks Administrator and the Nurse Manager. Current residents will be reassessed based on a change of condition, and this exam will be done at no extra charge by our nursing staff.
